This page provides instructions on how to create incident, non-conformance and hazard reports from the Lucidity App only. For more detailed information on adding new issues in general, refer to the Add New Issue page of the Lucidity Incident user guide.

Available Incident Records

Regardless of privileges users may have, only the initial entry details of an issue can be completed from within the App. Once saved it cannot be reviewed or edited in the App. Users can access the full Lucidity Incident module via a web browser from any device to view additional functionality associated with issues.

Create Issue

Lucidity App: Create Issues Reports

Currently, incident, non-conformance and hazard issue types can be entered via the App. The following explains how to enter an incident report but the steps are the same for any issue type.

To create a new incident, from the App Dashboard:

  1. Tap 'Add Incident' in the quick links



  2. Populate the initial details of the incident. Important points to note:
    1. Only the details, date of incident and time of incident are marked as mandatory
    2. Not all fields that are available via the browser version are available via the App
    3. This is to enable efficient and fast entry of the incident and for photos to be taken at the scene, under the assumption the report will be completed in more detail once a full web browser is accessible



  3. Select the relevant incident categories
    1. Note the actual incident category report page cannot be populated via the App. When selected, these act as a  placeholder and can be fully completed via a web browser



  4. Populate all other details of the incident as applicable
    1. Note if the Responsible and Closeout Managers are not selected, notifications will not be triggered
  5. Add any photos of the incident scene. Refer to the Shared Features page of this user guide for more details
  6. Tap 'Save' in the top right-hand corner to return to the Dashboard


Input Device

The Lucidity App is designed to be available for use offline and as an input device only. Therefore, in order to ensure large amounts of data are not saved to the device being used, once an issue has been entered and saved it is no longer accessible to view or edit via the app. If editing is required, the issue can be accessed via any Internet browser on any phone, tablet or computer.
