AUDITS

Audits

The Audits tab can be used to record details of audits carried out by third parties such as clients and regulatory bodies. 

 

External Only

It is recommended the Audits tab within Lucidity Incident is used to capture details on external audits only. It is suggested that Lucidity InForm is used for capturing details of internal audits as all checklists can be managed and configured, actions captured and attachments saved, allowing for much more flexibility and business control.

 

 Lucidity Incident allows the outcomes of the audits to be captured as either a:

  • Finding, or

  • Non-Conformance.

Both findings and non-conformances can be linked to actions. Actions can also be entered independently of a finding or non-conformance.

In the Audits tab of Lucidity Incident, users can (role dependent):

  • Add, edit and delete audit details

  • Print individual audit details

  • Report on audits in Excel format

Add New Audit

To add a new audit, from the Audit page:

  1. Click 'Add New'

     

  2. Complete the fields (Refer to 'Definitions of Fields' below)
    The details captured for each entry may vary slightly between each system configuration. Fields marked with * are mandatory.

  3. Click 'Save'

Definition of Fields

Entered By

Pre-populated to name of person currently logged in

Name of Audit

Enter a name or title for the audit

Date Carried Out

Use the date selector tool or enter the date the audit was carried out

Type

Select the type of audit

For example: audit, daily inspection, other

Internal or External

Select if internal or external

Note it is recommended Lucidity Incident is used for external audits only. Lucidity InForm is the recommended tool for all internal audits

Organisational Details

Select organisation details as applicable such as division, location, project etc.

Note location is listed as an open text field. This can be used to detail exactly where within the location the audit was carried out. The location may be the Millennium Falcon but the detailed description may be "in the kitchen area of the cafeteria on the Millennium Falcon."

Findings

Add any findings as a result of the audit

  1. Click 'Add'

  2. Select the individual the finding was identified by either from the drop-down list or by entering their name

  3. Select the finding type, for example finding, observation

  4. Enter the details of the finding

  5. Click 'Save' to save and close or 'Save and New' to enter an additional finding

Non-Conformance

Non-conformances as a result of the audit can be linked to existing non-conformances within the issues tab of Lucidity Incident

  1. Click 'Add'

  2. Chose 'Yes' to link to an existing non-conformance within Lucidity Incident already

    1. Select the non-conformance from the list

    2. All fields will be populated as per the existing non-conformance

  3. Chose 'No' to enter a new non-conformance

    1. Select the organisational fields

    2. Select the responsible and closeout managers

    3. Use the date selector tool to enter the date detected

    4. Enter a brief summary

  4. Click 'Save' to save and close or 'Save and New' to enter an additional non-conformance

Save and Continue

Click 'Save and Continue' to save data entered so far. 

Saving at this point also allows any actions entered to be linked to a finding created in the steps above

Actions

Add any actions required as a result of the audit

  1. Click 'Add'

  2. Enter the action description

  3. Select the actionee

  4. Use the date selector tool to enter the due date

  5. Select the status of the action as either 'Open' or 'Closed'

  6. Select the finding to link the action to where relevant

  7. Upload any files relevant for the actionee

  8. Click 'Save' to save and close or 'Save and New' to enter an additional action

Persons Involved

Add any person's involved in the audit

  1. Click 'Add'

  2. Select the person involved from the drop-down list or by entering their name

  3. Select the type of person, for example client, contractor, employee

  4. Enter the details of the person's involvement

  5. Click 'Save' to save and close or 'Save and New' to enter an additional finding

Contractor Involved

Add any contractors involved in the audit

  1. Click 'Add'

  2. Select all contractors involved from the list of values

  3. Click 'Done'

Supplier Involved

Add any suppliers involved in the audit

  1. Click 'Add'

  2. Select all suppliers involved from the list of values

  3. Click 'Done'

Log of Finding

Add to the log of findings as they arise

  1. Click 'Add'

  2. Use the date selector tool to enter the date detected

  3. Enter a file note

  4. Click 'Save' to save and close or 'Save and New' to enter an additional log entry

Upload a File

Upload any files such a photographs for the Issue

  1. Click 'Add Document'

  2. Locate the document

  3. Click 'Open'

  4. Enter the file name and description as applicable

  5. Click 'Upload'

Edit Audit

To edit an audit, from the Audit page:

  1. Use filters to locate the relevant audit

  2. Click 'Edit' on the far right-hand side of the audit

  3. Edit details as required

  4. Click 'Save' to save changes and return to the Audit page

Delete Audit

To delete an audit, from the Audit page:

  1. Use filters to locate the relevant audit

  2. Click 'Actions' on the far right-hand side of the audit

  3. Click 'Delete'

  4. In the pop-up window that appears, click 'Delete' to confirm

Reporting from the Audit Page

To print a PDF of all the audit details, from the Audit page:

  1. Use filters to locate the relevant audit

  2. Click 'Edit' on the far right-hand side of the audit

  3. Click 'Print' at the top of the page

  4. The PDF will open according to specific browser requirements

     

     

Excel List Report

To generate a report for a specific group of audits, from the Audit page:

  1. Select the relevant filters

  2. Click 'Filter'

  3. All results will be displayed at the bottom of the screen

  4. Click 'Excel Export' to generate this report into Excel
